How To Request Your Medical Records

It’s important that you have easy access to all your medical records. Staying informed about your diagnosis and treatment is a critical part of your care experience at Barrow Brain and Spine.

Step One: Completing Your Form Request

You will first need to complete the Barrow Brain and Spine “Authorization for Release of Protected Health Information” form.

Step Two: Delivering Your Request

Once you’ve completed the authorization form, you have three delivery options:

  1. Email it as an attachment to: NSAMedicalRecords@barrowbrainandspine.com.
  2. Fax it to (602) 264-2417.
  3. Deliver it in person to any Barrow Brain and Spine location during normal business hours.

Step Three: Awaiting Your Records

Due to the high volume of requests, expect to receive your records within 30 days. 

If you have an urgent request, please indicate that in your submission form. We are typically able to process urgent requests and route them to the necessary physician within a few days.

If you plan to pick up your results in person, please make prior arrangements with the Medical Records Department and bring a valid form of identification.
